Birth Preparedness Tools
Tools for Birth Preparedness Plus include several versions of birth preparedness cards and accompanying guides and record keeping forms. One card is designed to link families to community groups and health facilities. The set also includes a simple pregnancy wheel to calculate women's due dates.

Birth Preparedness Card A sample card designed to help mothers and families prepare for birth. (It should be printed or copied double-sided.)
Birth Preparedness Card (Linked) Another format for a birth preparedness card that has three "linked" parts. The first section is for the woman and family. The second section is for the women's groups who will support birth preparedness and the third section is for health care providers. (It should be printed or copied double-sided. When distributed, cut into three sections: Family, Community, and Clinic.)

User's Guides for a Birth Preparedness Card

Two step-by-step sample users' guides to accompany the birth preparedness card. One user's guide contains instructions only. The other also contains small replicas of the images from the card. These guides assist in training of community members who will use the birth preparedness cards.

A sample recordkeeping form for use by women's groups or other community members who will support birth preparedness. It is used to track all pregnant and recently delivered women in their community. (It should be printed or copied double-sided.)
